Add a comment to a phone call leg

Add a new comment to a call-detail record.

The comment is attributed to the authenticated caller (the creator is
resolved from the request token). phone_call_leg_id is validated as
a UUID and the body against the request schema — a malformed id, a
blank/missing value, or an unknown field fails with
422 at request validation. An unknown call-detail returns 404.
Returns 201 with the created comment (id + timestamps).

Request body for POST /phone_call_legs/{id}/comments.

Only the writable part is accepted: a required, non-blank value and an optional
create_mentions flag. calling (the softphone in-flight-call
placeholder behaviour) is intentionally not exposed — leaving it at its
default keeps the contract simple and guarantees that an unknown
call id returns 404 rather than silently creating a placeholder record.
Unknown fields are rejected (422) at request validation.

When true (the default), generate @-mention notifications from the body. Set false to skip them. Forwarded only when explicitly provided.
