Update a transcript phrase of a phone call leg

Correct one transcript phrase, returning the stored phrase as {"phrase": {...}}.

The phrase is addressed by order (+ phrase_type) in the body; the
phrase.id in the response is assigned on write and is not part of the
request. 404 if the call-detail id or the order is unknown.
